I understand IPTV is run by telcos, while digital CATV is operated by cable companies. But other than that, they seem to be pretty much similar.

IPTV is what Cisco Systems calls its video conferencing system.
As a general rule of thumb, video conferencing systems normaly use either the H.323 protocols, or SIP.
VOIP, or Voice over IP is utilized to carry voice communications over tcp/ip based media. There are two forms, Voice over IP, which is used primarily for transport and toll avoidance, and IP Telephony, which uses existing data networks, with trunks to the pstn for telco access, and provides all the features of a fully functional digital PBX system.
IP VPN systems are used to “tunnel” or separate traffic between client systems and most typicaly, corporate networks thereby creating a sort of logical “extranet”, allowing remote computers to operate as though connected to the Corporate LAN infrastructure. Most would view IP VPN’s as also being encrypted to prevent theft or access of the data when in transport, however encryption is not a requirement.
There are as many different technologies used to provide the above features as there are reasons for using those features. I would reccomend that youfirst determine you network requirements then from there you can choose the equipment you may need.

There is a huge difference:
IPTV provides TV services over IP. VoIP provides voices services over IP. IPVPN provides Virtual private networking over IP, mostly being a data service. They are all based on IP , but provide different services. The architecture of each is completely different from the other. The bandwidth requirements upstream and downstream direction are completely different, the IP mechanism being used in each other is completely different (eg IPTV uses heavily IP multicast, the other two being an unicast service only).
But, all technologies can be used at a customer at the same time. Crieterias are QoS mechanism, security capable IP network and end devices. The most important is that the provider has to provide, the delay, jitter, and bandwidth requirements at each service – and the security requirements also.
